Submit a request

Please note if your request involves data related to counselling case notes we will need
you to provide us with the following additional information in last field:
• Your home address at the time of using the service
• Your home and mobile phone number at the time of using the service
• Your email address at the time of using the service if different from now
• Your date of birth (yyyy/mm/dd)
• Approximate date when the counselling services were used
• Government approved photo ID, e.g. passport, driving license etc
